Medical Records

A medical release waiver must be signed in order to obtain a copy of your medical records or to have them sent to another health care provider. If you are requesting that your records be sent to another provider we ask that you supply us with their name, address, and phone number.

Prescription Refills

For prescription refills, please call your pharmacy and request that a refill authorization be faxed to our office at 615-377-8830.

This allows us the opportunity to pull your records to ensure that your medication type and dosage are accurate.

Please allow 24 hours for refills. Antibiotics will not be called in without seeing the doctor.

Referrals

If you require a physician referral, you must notify our office at least 2 days in advance of your scheduled appointment date.
